Job Purpose
The services provided by a Care Coordinator (CC) focus on helping individuals and family develop skills, access resources and learn to manage illness in order to be successful in the living, working, learning and social environments of their choice. CC’s teach, model and practice skills with person(s)-served in order to promote recovery and wellness.
Qualifications
- A mental health professional, OR
- Bachelor’s degree in social work, psychology, nursing or human services field, OR
- At least two years of higher education with two years of experience in psychiatric, substance use treatment or developmental disabilities; OR
- Any four-year degree with two years of experience in psychiatric, substance use treatment, or developmental disabilities; OR
- Four years of experience of equivalent experience with consumers and their families receiving psychiatric, substance abuse treatment or developmental disabilities services to move towards their personal, social, and vocational competency in order to live successfully in the community.
Core Job Duties
- Provide education to person(s) served to learn about and manage mental illness and/ or substance use disorder.
- Link person(s) served with community resources and help them to develop natural supports.
- Help person(s) served to develop or improve interpersonal, social, family, community and independent living skills.
- Provide transportation for the person(s) served when providing necessary interventions.
- Complete required documentation in a timely manner.
- Adherence to all applicable evidence-based practice models.
- Orally communicate information effectively and accurately.
- Assure program operates within the constraints of the agency’s certification (CARF, DMH) standards and strategic plan related to clinical care.
- Abide by program and agency policies and procedures
- Other job duties and special projects as assigned.
