POSITION TITLE: Fidelity Programs Case Management Position 

REPORTS TO:     Clinical Supervisor

FLSA STATUS:   1 FTE Non-Exempt Hourly

Salary: DOE



Position Summary:

Assists in the coordination of services as requested by a client’s Clinicians. May help coordinate services with various community support organizations including Primary Care, legal systems, community supports and relevant educational programs. May assist Crisis Team with follow-up care for those having utilized crisis services

Essential Duties and Responsibilities:

  • Gather information such as medical records, medication list, legal docs, etc. from various service providers to assist in the coordination of care for an individual.

  • Medication management which may include setting up pill boxes, dispensing medication under direction of QMHP, nurse or Physician. May provide heightened services during a crisis or in follow-up to the crisis increasing stabilization.

  • Make referrals and connect individual to necessary services as identified by the individual and team.

  • As individual is able, provide skill development and training to help ensure smooth transition between levels of care and to assist individual in remaining as independent as possible.

  • Assist individuals with recommended follow-up' services by connecting them to ride services, connecting them to child care services or other services that supports them in following through with recommended services and appointments with agency and other community service providers.

  • Assist individuals, prior to crisis or after stabilization of a crisis situation, with completing Mental Health Advanced Directives, helping them identify their wishes and desires should they go into crisis.

  • Other misc. duties as assigned.

 

Desirable Qualifications:

  • Some experience with and/or understanding of the special needs of persons who have a history of mental or emotional problems. 

  • Good writing and speaking skills. 

  • Past experience working in the human services field and ability to build relationships with individuals served and other professionals. 

  • General familiarity with mental health terminology. 

  • Other desirable personal characteristics would include tolerance, tact, the ability to control emotions in stressful situations and demonstrated functional maturity.

Experience and Training:

  • A Bachelors degree in a behavioral sciences field, or a combination of at least 3 years: relevant work, education, training or experience. 

  • Has the competencies necessary to communicate effectively; understand mental health assessments, treatment and service terminology and to apply the concepts; provide psychosocial Skills Development; and implement interventions developed with individual, mobile crisis team and identified in the Service Plan (within their scope of practice); and the ability to perform the duties listed under "Essential Duties and Responsibilities". 

  • Working with culturally diverse populations and speaking a 2nd language is a plus.

  • Must be familiar with office automation including computer and phone systems that requires fine manipulation, grasping, typing and reaching.

  • Has or is qualified and willing to obtain a QMHA certification through MHACBO

 

Work Environment:

  • Work is often performed in settings outside of the normal office environment to include other community offices, client's homes, jail, and the hospital. 

  • This position may be exposed to the everyday risks or discomforts which require normal safety precautions typical of such places as an office (i.e., moving mechanical parts, airborne particles, and electrical shock). 

  • This position may also be exposed to individuals and situations that are potentially dangerous. Employee will receive training on how to defuse situations and will receive information about agency policy around safety precautions and appropriate response to crisis situations.



CONFIDENTIALITY: This position requires the handling of highly confidential information. Must adhere to rules and laws pertaining to client confidentiality.

COMMUNICATION SKILLS: Must be able to communicate information in both oral and written form with the ability to present and exchange information internally across teams and co-workers, and externally with clients and the public, Information communicated ranges from routine/ basic information to complex and confidential information

LEADERSHIP & TEAMWORK:  Must be able to work effectively and constructively with other office staff, clinicians, the administrative team and all other staff. Participates in staff meetings. 

PHYSICAL DEMAND:  

This position requires you to be regularly available for work as scheduled. Position requires professional and personal skills to cope with stress associated with work involving a high degree of mental, emotional and physical demands. This position also requires the ability to bend, stoop, push and pull. While performing the essential duties of this job, the employee is regularly required to sit; talk and hear; use hands and fingers and handle or feel; reach with hands and arms.
