SUMMARY
Provides pharmaceutical care services to WCHC patients requiring specialized clinical pharmacy knowledge and practice expertise.
To perform this job successfully, an individual must be able to perform each responsibility satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability to be successful.
DUTIES/RESPONSIBILITIES (include but are not limited to the following)
- Provide pharmaceutical services to the Inpatient department (22 bed) and to the Out Patient Infusion Center.
- Evaluation of medication therapy, clinical assessment of patients, medication reconciliation, medication management, pharmacokinetic consults, resolution of emergent lab values, management of adverse drug reactions, therapeutic drug monitoring, and discharge planning.
- Dispense and compound medications, including parenteral, and/or supervises this process.
- Provide accurate, efficient and appropriate pharmaceutical distribution services to patients by properly interpreting providers drug orders and dispensing the appropriate medications.
- Communicate with after hours pharmacy service staff to ensure transition of care.
Qualifications
EDUCATION AND/OR EXPERIENCE
- Pharm. D. or B.S. from an ACPE-accredited College of Pharmacy, is required.
- An ASHP-accredited residency in clinical/hospital pharmacy, or equivalent experience, is highly desired.
- Current licensure as a pharmacist, by the Iowa State Board of Pharmacy, is required.
