Position: CSP Case Manager

Description of CSP Case Manager: The CSP case manager is responsible for providing supportive counseling as well as rehabilitative and social work services to persons with mental illness. They are also responsible for coordinating services with auxiliary programs, psychiatrists, other agencies, and the community. The CSP case manager is expected to serve around twenty clients on their caseload.
The CSP Case Manager duties and responsibilities:
Providing support services to clients which will include but is not limited to: outreach visits, supportive counseling, help maintained independent living skills and transportation of clients in personal vehicle when clinically indicated
Helping clients to remain responsible for their own lives by collaborating with them in developing their individual plan of care, help them maintain the skills they need to live in the community, maintain or obtain housing and income sources as needed
Will work closely with housing coordinator, linkage and intake coordinator to maximize access to housing opportunities and sources of income
Monitoring clients’ progress and response to medications to provide the agency psychiatrist with information useful in proposing effective psychiatric medications
Collaborating and consulting with other clinicians, physicians, hospitals, other caregivers, and families as clinically indicated
Providing a treatment plan every year, a clinical re-assessment every two years and a succinct monthly progress note for each client on the caseload
Advocating for clients when they are not capable of doing this for themselves
Attending required meetings and trainings, including individual and/or group supervision and other agency committee work when indicated
Providing your supervisor and other team members with the information necessary for them to ensure the efficient delivery of services
Completing all required paperwork on time
Fulfilling other duties appropriate to your position as assigned
Physical Requirements: The role may involve the following physical requirements:
Prolonged periods of sitting and standing during therapy sessions and administrative tasks
Effective verbal communication, including the ability to clearly convey information both in-person and over the phone
Basic manual dexterity to handle documentation and computer-related tasks
Mobility to move within the office environment and between different work locations

Qualification for CSP Case Manager: Masters Degree in related mental health field/social work preferred or Bachelors Degree in related mental health field/social work with a minimum of 1 year of experience. Supervision toward mental health/social work licensure provided.
