PROGRAM: Residential Support Services
POSITION: Residential Support Specialist (Supervised Apartment Program)
Description: A 40 position with benefits providing support services to consumers in their own apartments, ensuring that the assigned individual is healthy, safe, and actively learning within an apartment environment that fosters growth and choice. Support may include skill development relative to activities of daily living, social, residential, and leisure skill acquisition. The candidate must have flexibility in work hours to be able to support consumers with their scheduling needs to attend evening and weekend activities. Full time with benefits
Responsibilities:
- To support consumers in improving daily living skills and independent living skills to include but is not limited to:
*Assisting with meal planning, preparation and grocery shopping/making healthy food choices;
*Helping to promote an active lifestyle;
*Preparing for consumers weekly budgets and expenses
*Assisting consumers in community settings.
*Delivering and supervising the self-administration of medications according to agency guidelines
* Assist consumers with making and keeping appointments.
- Ensure consumers have the appropriate supports by:
*Collecting, compiling, and analyzing data to evaluate progress in meeting objectives indentified by individual and ISA team for any necessary modifications and record information and data (monthly progress notes, TCM notes) in a timely and detailed manner.
*Attending, participating, and contributing to the development of the ISA
*Providing assistance to consumers by attending medical/dental appointments to ensure that all information discussed with the consumer is interpreted properly, documented, and filed in the agency record
*Preserve privacy, confidentiality and insure dignity to assigned individual and all others.
*Possess the ability to work independently as well as with the team that supports the individual.
*Actively participate in staff meetings and supervision meetings
*To disseminate and follow up on instructions.
*Prepare and follow the schedule for consumers being supported while being independent and adaptable for scheduling changes that may occur
*Prepare for consumer medications on a weekly basis
*Organize transportation for consumers to attend various events
Qualifications: High School diploma with 3-5 years experience working with adults with Developmental Disabilities. Strong interpersonal and communication skills, demonstrate flexibility, and willing to work with consumers with a variety of support needs. Valid driver’s license, excellent driving record, and a safe, insured vehicle required.
