The Supervisor of Clinical Services assumes responsibility for coordination and management of Home Care services provided by the Agency to individuals and families, including Nursing, Therapy, Social Work, Dietary, and Paraprofessionals under the direction of the Home Health Administrator. Oversees all aspects of Paraprofessional Department, including direct supervision and responsibility for coordination of care (Schedulers). Provides staff education, mentoring and supervision of assigned professional staff. Functions as acting Home Health Administrator, when required.
Qualifications:
- Current license to practice as a professional registered nurse in the State of Connecticut.
- Three to five years registered nursing experience with minimum 2 years in Community Health/Home Care Nursing.
- Competent knowledge of Medicare Conditions of Participation and Department of Public Health regulations for home care.
- Two years of experience in supervisory or other administrative nursing positions Masters Degree in Community Health Nursing or related field preferred.
Responsibilities:
Home Care Services
- Provide patient centered leadership to Clinical professional staff
- Oversees scheduling staff in making patient and personnel assignments, coordinates patient care, assures that patient needs are continually assessed and assures the development, implementation and updates to individualized plan of care 24/7.
- Utilize the concepts of quality improvement to identify, implement, and follow-up for improvements in clinical practice, patient care, and process improvements.
- Use evidence-based models of practice and care.
- Responsible for oversight of the weekend/overnight/holiday clinical team including LPNs.
- Educate/mentor staff in management of complex/difficult cases.
- Interface with outside agencies, patient families, referral sources, and liaison staff as needed.
- Conduct and lead interdisciplinary team meetings on regular basis
- Conduct Quality Improvement activities for professional home care services through case conferences, OASIS review, Care plan (485) review and completion, clinical record review, billing review, on-site home visits (patient assessments), on-site staff supervisory visits and team meetings.
- Responsible for the successful orientation and preparedness of all new-hires under organizational supervision
- Review periodically with assigned clinician, outcomes of care, appropriateness of client goals, and achievement of client goals, and appropriateness and accuracy of care plan (485).
- Supervise and evaluate performance of all assigned staff to fulfill regulatory and accrediting requirements.
- Assist assigned staff in setting individual goals
- Assist assigned staff with intra and interagency relationships.
- Review staffing levels, make recommendations and implement changes.
Management
- Participate in Clinical Leadership
- Participate in long and short range planning.
- Participate in program planning, budgeting, and evaluation of activities related to clinical services of Agency.
- Share responsibility for identifying factors affecting health services delivery system.
- Identify problems affecting patient care and clinical management and develop solutions mutually with staff and administration.
- Provide for dissemination of information regarding Home Care Services to and from individuals and agencies outside agency
- Ensure professional development of staff and completion of necessary continuing education requirements through in-service education and provision of climate conducive to sharing of knowledge and expertise between peers.
- Participate in and involve staff in studies and research.
- Provide a climate conducive to educational experience for students.
- Communicate ideas/concerns of management to and from staff and elicit discussion.
- Assist with organizational changes incorporating procedures into daily action
- Assume leadership role of sub-committees and specific programs as appropriate.
- Participate in recruiting, hiring, and performance improvement plans of employees.
