Job Description
Position: Dentist
General Responsibilities: provide dental services to the residents that meets the law, regulations, and requirements of the contract for Mountain View Academy.
Duties:
· Dental services shall be provided in accordance with applicable law, license, regulation, contract requirements, and duly recognized standards of practice and professional ethics.
· Scope of services shall include but not be limited to:
Meet the dental needs for the residents of Mountain View Academy.
Provide on-call emergency dental services.
Other duties as agreed upon with the Facility Administrator
Contractor will pay Subcontractor monthly at the rate of $180.00 per hour for two (2) hours each week or a weekly payment of $360.00. Any hours above two (2) hours each week shall be paid at the hourly rate, herein, if those hours were approved in advance by the Facility Administrator. Subcontractor shall also provide emergency on-call dental services to be paid monthly at the rate of $300.00 per month. Subcontractor will not receive compensation during any period Subcontractor does not provide services, excluding any period of leave, provided Subcontractor provides Facility Administrator with a minimum of two (2) weeks advance notice in nonemergency situations and reasonable advance notice in emergency situations.
Qualifications: must have a Doctorate degree in Dental Surgery or Medicine.
