PRIMARY PURPOSE: To maintain safety during the overnight hours and to support the quality of life, independence and health of the individuals living in the assigned home.
LOCATION: ANDOVER MA
SCHEDULE FOR THIS POSITION: 40 hours, Mon-Thurs 11p-9a
KEY ACTIVITIES: Essential Functions
- Complete Individual Support Plan (ISP) goals and activities (Assist each individual in developing skills in all areas of community living, including training in self-care, household tasks, and socialization.)
- Ensure safety and provide help with personal care activities
- Administer medication when assigned
- Complete daily logs and notes, track behavioral data (Keep daily records on all teaching programs, activities and behavior; recording in medical log any medication given, seizures, and illnesses; recording in program log any incidents, events, and activities related to behavior plans in place.)
- Monitor individuals throughout the night and provide needed night-time care.
OTHER DUTIES:
- Coverage: Awake overnights must remain at the residence for all scheduled hours unless PD grants permission to leave.
- Money management: Assist individuals with preparing and documenting spending money according to personal budget plan.
- Behavior: Assist and guide each individual to behave appropriately in varying situations; as agreed in any behavior plans, formal or informal, in place.
- Nutrition: Awake overnights will provide for breakfast and packed lunches on weekdays, when applicable, and with appropriate concern for good nutrition, variety, individual choice, and prescribed diets.
- Home management: Complete assigned overnight chores and assist individuals with completing morning household tasks such as cleaning the kitchen and laundry. Report any needed program repairs.
REQUIREMENTS:
- High School diploma or GED
- Reading, Writing and Math Skills at high school competency level.
- Must have MAP Certification or be certifiable.
- Valid Drivers License.
- Experience in providing training, case management and appropriate service delivery to adults with intellectual and developmental disabilities living in the community.
- Minimum of 1 year of driving experience and a valid driver's license
- Regular and predictable attendance
