What are the responsibilities and job description for the Concierge position at We Care HCC?
As a Skilled Nursing Facility Concierge, you will play a pivotal role in ensuring the smooth operations of our facility by providing exceptional customer service to residents, families, and staff. The concierge will work closely with the Director of Admissions, housekeeping, and Maintenace to streamline the appropriate communication and guidance to the facilities IDT. This position requires a friendly demeanor, excellent communication skills, and a proactive approach to problem-solving.
Responsibilities:
· Greeting residents upon arrival and providing a warm welcome.
· Room readiness for any new admissions, and readmissions. This can include but is not limited to:
o Coordinate with housekeeping and maintenance staff to address any room-related issues.
o Conduct room inspections to ensure cleanliness and appropriate equipment is in place.
o Place welcome bags in the room for every new admission.
o Place welcome sign on the door for every new admission.
· The completion of Admission Agreements with the resident and/or responsible party.
· Relay important information, and updates between departments to ensure seamless coordination.
· Email notifications for any new admissions and readmissions to the IDT/UR teams with outlined detailed admission information including appropriate medical information.
· Maintain an accurate and up-to-date room roster.
· Inputting new admissions to the facilities EMR system.
· Collecting and uploading appropriate documents to EMR.
· Run Coverage Eligibility verification and upload into EMR.
· Ensure all HCP/POA documentation is assigned to appropriate contact and uploaded into EMR. (If available)
· Update contact information after receiving signed Admission Agreement.
· Other job duties as assigned.
