What are the responsibilities and job description for the Onboarding Care Manager position at WeCare 365-Adult Foster Care/Adult Family Care?

REQUIREMENTS/QUALIFICATIONS per AFC Manual:

Care Manager to provide psychosocial and counseling assistance for the AFC program.

Qualifications: The Care Manager must have

  • Demonstrated leadership ability.
  • Bachelor’s degree
  • Care Management experience
  • Excellent verbal and written communication skills
  • Excellent computer and technology skills
  • Meets consistent level of excellence with performance as Care Manager
  • Commitment to quality care, and the multidisciplinary team approach
  • Competence in CPR and First Aid
  • CORI, Physical in the past 12 months, and Annual TB Test
  • Reliable Transportation
  • Ability to drive to multiple locations within Massachusetts to meet required visits.

SUMMARY OF RESPONSIBILITY:

Performs intakes and contract signings for new and reenrolling members to adult foster care program.

PERFORMANCE CRITERIA:

  • Initial assessment of AFC qualified setting. Including completion of Environmental Inspection tool
  • Informing members of and referring them to available community services
  • Strong commitment to the right and ability of each person with a severe and persistent mental illness to live in normal community residences; work in market jobs; and have access to helpful, adequate, competent, and continuous support and services.
  • Excellent verbal, written, and interpersonal communication skills.
  • Completing care management progress notes
  • Adherence to deadlines
  • Completion of assigned projects
  • Allows new Care Managers to shadow visits and be available to answer questions.

WORKING CONDITIONS and REQUIRMENTS per We Care 365:

  • Frequent commuting in automobile up to 95%
  • Free standing patient environment with exposure to clinical equipment/machinery, medications, cluttered rooms and hallways, noisy environment with frequent interruptions.
  • Exposure to infectious illness, body fluids and cleaning materials.
  • Office and technical equipment such as computers, fax machines and copy machines.

Duties and Responsibilities include but are not limited to:

  • Provide respectful engagement to adults with histories of chronic illness, disabilities, and co-occurring clinical needs and psychiatric disorders.
  • Build trust with each family, and conduct a non-judgmental, strength-based assessment of the family’s housing situation and overall strengths and needs.
  • Connect Caregiver and Member with needed resources and services and follow-up with the family service plan during the onboarding process.
  • Enter all data into including but not limited to case notes, assessments, and care plan outcomes, and other database systems utilized by the We Care 365
  • Function as liaison to and consult with We Care 365 partners, community agencies, programs.
  • Document all engagement and case notes for each client.
  • Work with Supervisor to develop best practices that can be replicated with other clients.
  • Attend on-going internal and external trainings on topics which include but are not limited to housing services, cultural competency, trauma informed care behavior support, First Aid and CPR, HIPAA and incorporate these techniques into the approach with families.
  • All other duties as assigned by supervisor.

DAILY/SPECIFIC ROLE REQUIREMENTS:

We Care Outlook Email, Outlook Calendar, and Database System (4D)

  • CM is responsible for checking their We Care 365 Outlook Email every 2 hours and responding within 4 hours and marking that they have received it.
  • Document all appointments in task in your shared calendar to reflect the changes made to the appointments you have missed. You will also utilize Database System to document dates of visits for MassHealth Reporting. 
  • All Outlook appointments must include the Caregiver’s name, Member’s name, and case ID number. Address, phone number and type of visit.
  • Using the database system, please include any information you receive on rescheduled appointments or replacement of care manager for the appointment, missed appointments and contact attempts.

Case Files – Chart Review

  • CM is responsible for checking each of their Case Files one or more times per month as a Chart Review for completion and compliance. This is done through the Database System. Scan for documents missing from the file (PSF, MDS, Logs, etc.)
  • Confirm documents are signed.
  • Confirm visit completion.
  • Confirm 3 documented tries for missed visits.
  • Confirm all visits have a date in the system.
  • CM responsible for supporting We Care 365 by reinforcing the consequences for infractions to the contract that the Caregiver has signed to provide care to the Member. CM needs to be comfortable in articulating that the requirements of the program and our ability to provide a stipend are regulated by MA Adult Foster Care program, not We Care 365.
  • CM should follow up with Caregivers and Members reminding them of the requirements to keep and attend all medical appointments and schedule for yearly Physicals and bi-yearly TB tests.
  • CM is responsible for Semi-Annual and Annual Fire Evacuation Drills.
  • CM is responsible for presenting and reviewing Evacuation Safety Plan to members and caregivers within one month of coming on to the We Care 365 program.
  • CM is responsible for Special projects assigned by Supervisor, Manager or Director

Job Types: Full-time, Part-time

Pay: $55,000.00 per year
