Job Description
WelbeHealth PACE (All-Inclusive Care for the Elderly) program provides seniors with the opportunity to continue living in their homes and in their communities. Our innovative and comprehensive range of medical services to participants is what ignites our passion to treat the whole person and not the symptoms! We employ a collaborative interdisciplinary team (IDT) approach to evaluate and guide participant care, which is key to WelbeHealth values, team culture, and mission.
\nThe Medical Director performs in-person comprehensive assessments and coordinates with the IDT to develop a care plan for each participant. At the direction of the Medical Director, the Associate Medical Director oversees all care of participants in nursing homes and hospitals including admits and discharges for acute, short-term, and long-term placement and the use of medical specialists to ensure effective care coordination. The Associate Medical Director also provides direction and support to Nurse Practitioners and other members of the IDT.
\nEssential Job Duties:

- Perform comprehensive assessments for participants and coordinate with the IDT to develop a comprehensive care plan for each participant \n
- Be an engaged member of the IDT, communicating participant changes, collaborating on care planning arrangements, and coordinating 24-hour care delivery \n
- Work collaboratively with network providers and other members of the IDT to ensure effective care coordination, particularly around hospital discharge, short-term, and long-term placement \n
- Provide direction and guidance to Nurse Practitioners \n
- Oversee participants’ use of medical specialists and inpatient care \n
- Participate in rotating night and weekend call \n
- Oversee all care of participants in the nursing home, providing regular visits as dictated by PACE standards and participants’ needs \n
- Review outcome and utilization data, including adverse events, hospitalizations, and use of specialists, in collaboration with the IDT to identify opportunities for improved care and operations \n
Job Requirements:

- Successful completion of a residency program accredited by the Accreditation Council for Graduate Medical Education \n
- Master’s degree/doctorate degree \n
- Board Certification – Internal Medicine or Family Practice \n
- Board Certification – Geriatrics preferred \n
- Active California Physician License without restrictions \n
- State CDS Certification and DEA Registration \n
- Valid state driver’s license with a clean DMV record \n
- Copy of recent vehicle insurance required \n
- Excellent clinical, organizational, and communication skills in settings with frail, elderly participants, their families, and the IDT \n
- Current knowledge of chronic care/geriatric issues and best practices \n

Salary/Wage base range for this role is $261,546.00 - $326,932.50 Bonus Equity. WelbeHealth offers competitive total rewards package that includes, 401k match, healthcare coverage and a broad range of other benefits. Actual pay will be adjusted based on experience and other qualifications.
