Job Summary

WELLBE INTRODUCTION

The WellBe care model is a Physician Led Advanced Practice clinician driven geriatric care (care of older adults) team focused on the care of the frail, poly-chronic, elderly Medicare Advantage patients. This population is typically underserved and very challenged with access to care. To address these problems, we have elected to bring the care to the patient, instead of trying to bring the patient to the care. Care is provided throughout the entire continuum of care – from chronic care and urgent care in the home, to hospital, to skilled nursing facility, to assisted living, to palliative care, to end of life care. WellBe's physician/advanced practicing clinician led geriatric care teams’ partner with the patient’s primary care physician to provide concierge level geriatric medical care and social support in the home as well as delivering and coordinating across the entire care continuum.

 

GENERAL SUMMARY

We are looking for an individual with deep transformational change experience in community-based care who is looking for an exciting challenge to join WellBe’s Performance Management Team to help transform the organization through data-driven process improvement.  This is a high-visibility, “in-the-trenches” role and will include daily interaction with our Performance Management Team, field operations, IT and analytics teams. You will be expected to leverage your transformational change expertise to help identify opportunities to minimize defects and improve process outcomes, while demonstrating an ability to produce executive level deliverables and solid judgement when solving problems.

 

Some of your work products will include financial analyses, process performance reports, process flows, group meeting facilitation, and interdepartmental relationship development. The ideal candidate will be a high-performing hands-on individual who has proven delivery with a strong understanding of operations and technology. You will have excellent communication skills, attention to detail and the ability to form the big picture. You likely thrive under pressure and see ambiguity and uncertainty as your opportunity to solve problems, innovate and deliver.

 

You will be expected to work both independently and as part of a team within a highly cross-functional environment. IT is critical the Performance Management Team serves as the connective tissue across the organization to drive true cross-functional collaboration and alignment while achieving lasting change. Job Description

POSITION RESONSIBILTIES

  • Actively build and participate in the operational rhythm of the business including appropriate communication, insight and incident management
  • Partner across teams and with key stakeholders to simplify processes, policy and align procedures to changing business models/priorities
  • Leverage real-world understanding of community-based care processes and be an expert and trusted agent of communication to drive both accountability and prioritization of critical operational processes
  • Additionally, you will use data to generate key insights including variances to best practice, current maturity and performance, adoption levels, and pain points
  • Ensure initiative tracker, action registry and meetings are driving top priorities and work down plans are created as appropriate to track progress
  • Collaborate with field operations team to optimize efficiencies around daily operations adhering to compliance requirements while reducing time away from primary tasks for frontline staff to drive process and performance improvement
  • Ensure performance expectations are communicated, measured and reported timely
  • Run weekly Analytics SWAT to triage metric outcomes, assess trends and derive actionable insights
  • Run daily/weekly metric cadences with market leadership teams
  • Drive overall routine hygiene; work with markets to ensure adherence to best practices and role requirements
  • Run governance cadences to provide strategic oversight for the market
  • Work with IT, Operations, and Medical Management to build out standard workflows
  • In collaboration with other Performance Management team members, identify and share best practices across markets
  • Memorialize standards, SOPs, policies, etc.
  • Support markets in roll-out of new initiatives (e.g., standards, workflows, etc.)
  • Serve as a mentor and resource for the operations team
  • Serve on new market LAUNCH team to deploy the “WellBe Way” in newly opening markets

Job Requirements

QUALIFICATIONS

 

Educational/Experience Requirements:

  • Bachelor’s degree in healthcare administration, business operations, or other related educational field.
  • Master preferred
  • Experience in population health/community health/managed care programs strongly preferred
  • Experience with Health Plan Medicare Advantage products
  • 7  years with a strong emphasis on community-based care, operations, finance and data analytics

Travel requirements: 25% local or national travel required

 

Work Conditions:  This position will work in a variety of settings, e.g. office. The noise level in the work environment is usually moderate.   Requires prolonged sitting.  Requires bending, stooping, twisting, kneeling, crouching, crawling and/or stretching from seated or standing positions. Requires eye-hand coordination and manual dexterity sufficient to operate medical equipment, frequently operates a keyboard, telephone, copier, calculator and other office equipment.  Manual dexterity and coordination necessary to operated office equipment, telephone, keyboard, copier and calculator.  Requires close vision, peripheral vision, and ability to adjust visual focus, hearing and smelling.  Must be able to communicate information via telephone or computer.  Requires moderate to intense concentration due to complexity.  Must be able to lift and/or move up to 25 lbs.
