Job Purpose:
Our next Care Navigator will provide peer support to persons' receiving services through the Clinic. They provide psychoeducation on behavioral health recovery, coordinate services and referrals to other service providers, and offer self as a mentor and role model of recovery.
Duties:
- Provide telephone brief screenings of clinical needs, and funding sources, to individuals and families requesting outpatient services, documenting in EHR.
- Participate in initial treatment plans, strengths and culture preferences, and intakes as needed.
- Address barriers to care or referrals and is resourceful in problem solving.
- Participate in hospital and emergency department discharge processes to transition consumers to a safe community setting according to CCBHC protocols. Ensure timely transfer of medical records, prescriptions, and active follow-up.
- For consumers at risk for suicide, coordinate consent and follow-up within 24 hours and implement a continuing plan for suicide prevention and safety and linking to services.
- Establish linkages and partnerships with community resources such as housing, social services, education, and employment to facilitate wellness and recovery of the whole person.
- Establish positive working relationships with other Wellpoint Care Network programs and sites to ensure efficient client referral and coordination across continuums of care.
Qualifications:
- Bachelor’s degree in social work, Psychology, Community Counseling, or related field preferred
- High school diploma required.
- Bilingual in Spanish preferred.
- Willingness to work beyond normal working hours: evenings, nights, and weekends.
- Maintain accurate and timely collaborative documentation.
- Demonstrates ability to work independently and effectively with staff, clients, caregivers, and purchasers.
- Must be honest, dependable, and able to meet deadlines; Self-motivated and able to work independently.
- Valid driver's license with an acceptable driving record, reliable transportation, and meet agency auto insurance requirements.
- Regular travel in personal vehicles is required with the ability to move intermittently throughout the workday.
- Ability to demonstrate critical thinking skills, detail oriented, highly effective verbal, and written communication skills.
- Demonstrate an ability to effective understanding and practice of cultural humility and ability work and communicate respectfully with individuals from diverse backgrounds.
- Proficient in the usage of computer software; demonstrate an understanding of computer file systems and databases, e.g., Microsoft Office, Excel, Outlook, Teams, ADP, etc.

- Public Service Loan Forgiveness (PSLF) - Wellpoint employees may be eligible for loan forgiveness under the Public Service Loan Forgiveness program.
- Interaction with personnel, clients, family members, visitors, volunteers, interns, and vendors under all conditions and circumstances.
- Physical exam, drug screen, motor vehicle report, and background checks are required for this position.
