Job Description:
Under the general supervision of the Clinical Manager and works in collaboration with the primary care providers and all members of the health care team. Primary responsibilities include identification of Medicare patients appropriate for Annual Wellness Visits (AWV) and execution of the guided processes to complete the AWV. Assesses healthcare needs that require additional follow up by primary care provider and assists patients in scheduling appropriate follow-up. This position requires expertise in the nursing process, using critical thinking skills, plan and coordinate care. Responsible and accountable for direct and indirect patient care for designated patient populations as a member of the clinical team. Must be an innovative thinker and open to changing the way we deliver care. Duties may include quality monitoring, patient assessments, direct patient care, and patient/staff education. Provides safe, therapeutic, efficient quality care and services to all patients served. Functions as a member of the health care team at licensed nurse level to assist in the planning and implementation of patient care, based upon sound nursing judgment, clinical preparation, knowledge, skills, and experience.
Duties and Responsibilities
- Review office schedules and identify patients appropriate and due for Medicare AWV.
- Work collaboratively with clinical teams to accomplish AWV in an efficient manner for patient and team.
- Follow AWV protocols and standing orders including completion of preventive services that may result from AWV.
- Develop and share knowledge of AWV eligibility and documentation requirements.
- Develop knowledge of Medicare preventive services and clinical guideline recommendations for those serves.
- Address open quality gaps with patients and works collaboratively with patient towards closure of those gaps.
- Utilize appropriate protocols and standing orders in delivering patient care.
- Participate in regular team meetings, huddles, staff meeting and quality improvement projects to improve patient care.
- Maintain annual competencies for role.
- Document all communication with patient in electronic medical record.

- Minimum Experience:
- 1-3 years
- Minimum Education:
- Completion of LPN program
- Required Certification:
- Registration and current LPN license in PA; BCLS certification
- Physical Demands:
- Prolonged periods of standing/walking
- Occasional sedentary work
- Lifts supplies, files and equipment
- Occasionally lifts, positions, pushes and/or transfers patients
- Periodic reaching, stooping, bending, kneeling, crouching
