Job Description:
Under the general supervision of the Home Health Clinical Manager, provides professional nursing services to individuals in the home through a process of nursing assessment, planning, care, and evaluation in accordance with physicians’ orders, the established plan of care, the patient’s diagnosis, corporate policy and procedures and the Nurse Practice Act. Carries primary care management functions for assigned patients. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. May require specialized skills depending on VNA assignment.
Duties and Responsibilities
Essential Accountabilities:
- Independently recognizes subtle changes in condition and modifies plan of care accordingly. Safely and independently administers prescribed treatments, medications, and nursing interventions based on assessment and plan of care. Initiate’s emergency action in response to acute risk situations.
- Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload.
- Evaluates patient care and promotes goal-directed change to achieve patient outcomes. Able to discuss, document and apply Performance Initiatives and outcomes at the VNA level as they relate to the WellSpan Bold Goals.
- Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients. Initiates interdisciplinary collaboration in evaluation of patient progress. Identifies and facilitates individualized patient needs for transfer/ discharge and includes patient/family in decisions.
- Begins reading current literature. Express interest in evidence-based research and its application to best practice. Understands clinical relevance and its impact on patient outcomes.
- Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired. Required education and competencies completed, including state mandated continuing education credits for licensure renewal. Encouraged to share pertinent information and job knowledge, related to readings, educational programs, and participation in professional organization activities.

Common Expectations:
- Demonstrates knowledge of the principles of growth and development over the life span of the assigned patient population. Assesses and interprets patient age specific data and provides appropriate, age specific treatment. Provides direct patient care to assigned patient age group(s).
- Demonstrates a commitment to patient, visitor, and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and "error proof" systems that will lead to increased safety; and participating in safety education programs and root cause analyses as required.
- Enhances professional growth and development through advanced/continuing education to maintain up-to-date knowledge, skills, and competencies.
- Maintains established policies and procedures, objectives, quality assessment, safety, environmental and infection control standards.

Qualifications
- Minimum Experience:
- 1 year of previous experience with the same patient population OR current VNA LPN transitioning to RN
- Minimum Education:
- Diploma or Associates Degree in Nursing, RN
- Preferred Education:
- Bachelor of Science, Nursing (BSN)
- Required Certification:
- Registration and current RN license in PA; Basic Life Support (BLS) certification required
Salary : $59,900 - $75,900
