What matters to us:
- Must have High School diploma or G.E.D.
- Per state regulations, must be at least 21 years of age
- Possession of valid driver’s license and insurable driving record
- Understands the individuals’ rights and displays respect and dignity when interacting with service recipients. Becomes knowledgeable of individual’s objectives, preferences, diet consistencies, rights restrictions, and individualized crisis plans.
- Ensures safety and well-being of individuals and implementation of individual’s support and/or behavior plan.
- Conduct on-site visits to each FHP home to monitor regulatory compliance.
- Collects monthly paperwork.
- Provides supplies to participant’s when needed.
- Ensure the house monthly books are complete.
- Ensures residential programming developed during the participant’s Person Centered Service Plan meeting is implemented.
- Ensures that homes are clean, well-kept, and stocked with all needed supplies and forms. Reports all maintenance issues as needed.
- Ensures FHP residential monthly summaries are completed by established deadline.
- Ensures each FHP participant has money in their home and all spending is accounted for, if needed.
- Monitors and reviews documentation to ensure pertinent information regarding the individual (i.e. behavior, special events, medical status) is current, objectives and outcomes are being documented, MAR’s/medical record documentation is complete, and money accountability logs are recorded and accurate.
- Complete HRST updates and renewals on all FHP participants as needed.
- Perform other duties as assigned by supervisor.
