What are the responsibilities and job description for the Memory Care Manager position at WesleyLife?
Our RN/LPN Household Coordinator supervises and coordinates the household teams to ensure the delivery of the best quality health care for the residents living in our memory care area. Our RN/LPN Household Coordinator will:
- Assess resident needs and prepares and maintains Service Plans, in collaboration with families and other services.
- Assist in the evaluation of the health care services needed for each resident relative to the level of lifestyle accommodations. Communicates and counsels with residents, family members and Director – Assisted living regarding potential problems, needs, and care plans as appropriate.
- Orient, train, and provide ongoing training and supervision for team members. Ensures orientation conducted for each employee. Provides input/develops performance evaluations and recommends actions on employee complaints, resolution, terminations, etc. Assists in monitoring employee progress and provides feedback to the Director – Assisted Living as necessary. Makes recommendations to the CHA for policies and procedures that govern the day-to-day operations.
- Share on call responsibilities every 3rd weekend
- Oversee the scheduling practices for the John’s Harbor Memory Care, working with the assisted living scheduler/Director – Assisted Living.

Experience
Required- RN or LPN licensure in the State of Iowa
- 2 - 5 years: Management Experience
- 2 - 5 years: Dementia/Memory Care experience
