We are seeking a Clinical Case Manager for our Residential Program. In this role you would provide supervision and support to clients living in a group home or apartment/community setting, while encouraging residential client’s wellness and recovery through group activities, education in daily living skills, supportive counseling and medication management. As a Case Manager you would participate in the development of Treatment Plans for each client on caseload as well as providing over sight and coordination of clients receipt of medical care benefits and all Psychiatric rehabilitation services.
This is a full time position. Hours of work are 4pm - Midnight, Monday - Friday.

Qualified applicants must meet one of the criteria described below to be considered for this positions.
Minimum Qualifications: BA degree from an accredited college or university with a major in a mental health, health, or other appropriate human services related discipline; or a licensed registered nurse; or a combination of one or more years of college plus related work or life experience together equaling four years (The individual is required to have at least one year of related work or life experience, if the four year college education does not meet the requirements above); or a license as a practical nurse plus two years of related work or life experience; or a high school diploma or the equivalent, plus four years of related work or life experience. _ Additionally one year experience in a residential mental health setting is required._ CPR/First Aid trained, to be obtained prior to providing services alone and within 30 days of hire.
Preferred Qualifications: The above and a valid drivers license. Good driving record, meets recommendations of Agency Automotive Insurer
