West Coast Hospice Management, Inc. is seeking for a reliable, caring, compassionate and responsible Registered Nurse (RN) to join our team. Under the guidance and direction of our Medical Director and working collaboratively with the other members of the Interdisciplinary Hospice Team within the Hospice Agency, the RNwill be responsible for conducting scheduled assessments and the admissions of assigned patients, which includes coordinating, establishing, and maintaining the patient’s plan of care. Provide staff supervision visits and follow up visits to monitor, observe, assess, and implement a pain/symptom management program to enhance the quality of life, and provide the highest level of hospice care to our patients. The RN will submit appropriate documentation and reporting requirements in timely manner, as well as communicating the orders and relay necessary actions to assure coordination of services, including medication refills, and other needed services or request by patient like podiatrist, wound care specialist, and therapy. The RN will provide guidance, advice and emotional support to patients and their family members.
Duties:
· Performs comprehensive nursing evaluations.
· Assesses for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action.
· Initiates plan of care and provides appropriate nursing follow up to all patients.
· Instructs caregivers in managing patient needs.
· Coordinates all services provided to patients / families in all settings to ensure quality of care and services as well as appropriateness of care.
· Communicate with collaborating physicians or specialists regarding patient care.
· Educate patients about health maintenance and disease prevention.
· Ensure plan of care problems, interventions, actions, and goals reflect baseline, current, and ongoing status, and changes as they occur.
Position Qualifications:
- Current RN license in California
- Valid driver's license, auto insurance, and reliable vehicle
- CPR/First Aid Certified
- Provide current Health/Physical Exam including TB test result
- Minimum of One Year experience in hospice, home health, clinic, or health facility
- All candidates require the ability to work autonomously and in a team setting
- Excellent observational, clinical, written, and verbal communication skills
- Understands Hospice philosophy and is comfortable providing specialized care to the terminally ill
- Good organizational skills and customer service skills
