Special Education Facilitator - Temporary for School Year 2023/2024
Performance
Responsibilities:
1. Functions as LEA Representative and coordinator of building level special education
teams including referral, evaluation, identification, IEP service delivery, and placement.
2. Acts as LEA representative who is qualified to provide, or supervise the provision of,
specially designed instruction to meet the unique needs of children with disabilities; is
knowledgeable about the general curriculum; and is knowledgeable about the
availability of the resources of the District.
3. Maintains school building compliance with New Hampshire Rules for the Education
of Children with Disabilities.
4. Maintains knowledge, as required by District, of Federal and State Special Education
Laws and Rules.
5. Facilitates data collection and maintain reports and requested information to the
Student Services Department.
6. Responsible for assuring that all Special Education paperwork is completed in an
accurate and comprehensive manner.
7. Acts as liaison to court by completing and submitting appropriate paperwork.
8. Responsible for overseeing Medicaid reimbursement/documentation forms in the
building.
9. Coordinates Extended School Year Program referrals.
10. Coordinates transitions of educationally disabled students’ across grade levels,
building programs and out of district placements.
11. Assists with diagnostic assessment of students (i.e., completes classroom
observations and achievement tests) to assist in maintaining timeline compliance.
12. Coordinates District consultants, evaluations, reporting and liaison to teams (i.e.,
psychologist, behavior consultant, ASSETT Services, etc.)
13. Acts as a liaison between school and Department of Student Services.
14. Provides leadership to Special Education Teachers, Special Education Teaching
Assistants, and related service providers re: special educators’ roles, classroom
modifications, behavior management, data collection, etc.
15. Contributes to teams in developing inclusionary programming strategies/ curriculum
adaptation and modifications for students with special needs.
16. Assists with specialized instruction for students with disabilities
