Nursing Case Manager
Health Services Department
Duties:
-Responsible for identification, organizing and coordinating resources and services to meet healthcare needs of high-risk students across the district
-Work in collaboration with students, family/caregiver(s), district staff, clinic/hospital/specialty providers and community resources to create a team approach
Qualifications, Education and Experience:
-RN licensure in the Commonwealth of Massachusetts
-BSN required
-DESE licensed as a School Nurse or eligible for licensure
-Current Healthcare Provider CPR/AED certification required
-Must have reliable form of transportation
-One (1) to three (3) years’ experience in case management, pediatric home care, and/or school nursing required. Strong demonstrated knowledge of community health services required.
-Excellent interpersonal, organizational and communication skills and ability to collaborate effectively
Salary: Per MNA Contract
