What are the responsibilities and job description for the Mentor Team Coordinate & Care Manager position at WestBridge Inc?
The Community Integration Program is made up of a trans-disciplinary team of Care Managers that provide treatment interventions to individuals who experience severe and persistent mental illness with or without co-occurring substance use disorders.
The Mentor Team Coordinator is responsible, personally or through appropriate delegation, for maintaining the structure of the mentor program, training and supervision of mentor team members, and promoting an environment of recovery that embraces accountability and authentic communication. The Mentor Team Coordinator works in conjunction with the Care Management Team Coordinator and Clinical Director to oversee the support provided by the Mentor Team and collaborates for continuity of participant care. Also serving as a Care Manager on the Community Integration team, the Mentor Team Coordinator provides supportive services to participants and families as directed by their Personal Achievement Agenda.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
As Mentor Team Coordinator:
- Provides supervision to the mentor team to support the delivery of mentor services within scope of practice.
- Monitors Mentor documentation to ensure it’s in accordance with organizational standards regarding timeliness and content.
- Supports hiring, onboarding, training and professional development of Mentor Team.
- Works collaboratively with Community Integration Program team to ensure coordination of services.
- Oversees administrative mentor team tasks including scheduling, timecard approvals, time off requests, expense reports, squire review, etc.
- Identifies areas for improvement in Mentor services and brings concerns to the Leadership Team to problem solve, identify solutions and develop implementation strategies.
- Maintains compliance with local, state and federal regulations, including but not limited to HIPAA and 42 CFR part II.
- Adheres to and upholds all company policies and procedures.
As a Community Integration Program Care Manager, meets a direct care (documented) productivity standard of 25%:
- Clinical duties could include but are not limited to, intake, evaluation, ongoing assessment of mental status, individual supportive coaching, co-facilitation of psycho education groups, co-facilitation of family education and support and clinical updates, drug screening, crisis intervention, and case management for individuals and their families. Services are provided collaboratively amongst a multi-disciplinary team, under clinical supervision and based on an individual’s scope of practice as identified by WestBridge or relevant licensure.
- Point Person responsibilities include but are not limited to: advocacy around individual needs for treatment as identified in Personal Achievement Agenda (treatment plan), monitoring and collaborating around mutually agreed upon service level options.
- Participates in the clinical on-call rotation and collaborates with the team to provide weekend and holiday coverage.

- Knowledge obtained through completing a Bachelor's degree curriculum in an appropriate discipline from an accredited institution or equivalent combination of education and experience.
- Minimum of 2-4 years working in community-based treatment, preferably as part of an ACT team.
- Skill in client assessment, including the use and withdrawal from substances, change in psychiatric status, physical health, and safety.
- Knowledge and experience in the treatment of mental illness and substance-use, preferably co-occurring. Understanding of stages of change and experience in the delivery of evidence-based practice desired.
- Knowledge and skill in maintaining care that is trauma-informed and recovery-oriented.
- Skill and experience in staff management, ability to direct staff, and provide supervision.
- Knowledge of state and federal rules and regulations governing confidentiality.
- Skill in working and communicating with others. Participates in open, direct, solution-focused communication with participants, families and team members.
- Demonstrates a high standard of professional conduct, professional boundaries and ethical behavior that will enhance the quality of care and encourage positive interactions among staff, participants and families.
- Valid driver’s license and appropriate vehicle insurance (liability limits of at least 100,000 bodily injury per person /300,000 bodily injury per accident/100,000 property damage).
- Satisfactory Motor Vehicle Records results.
- Employees are responsible for maintaining their personal vehicle and equipment in a safe, operable, & clean state at all times, driving it in a safe and professional manner.
- Satisfactory Criminal Background Screening results.
- Computer skills including but not limited to, Internet access, word processing, videoconference, electronic medical record, report writing and spreadsheet applications
- Successful pre-employment drug screening with negative results required.
- Active CPR, First Aid and AED, and Crisis Prevention Intervention (CPI) Certification required or a plan for timely completion upon hire.
