We are seeking a passionate and knowledgeable Lifestyle Coach to join our Fortified Life team. As a Lifestyle Coach, you will play a pivotal role in fostering a culture of wellness and empowering our members to lead fulfilling lives. The ideal candidate will have a background in wellness and nutrition, along with a deep understanding of the dimensions of wellness.

Responsibilities:
- Develop and implement personalized wellness plans for residents, focusing on physical, emotional, social, intellectual, occupational, and spiritual dimensions of wellness.
- Carries a caseload of 40-50 members within Fortified Life/Wellness Programming.
- Provides Lifestyle coaching (i.e. one on one coaching weekly, bi-weekly, or monthly, designed to keep members successful through all areas of well-being) to members based on their needs throughout their time in the program.
- Build relationships with members to effectively help improve their well-being, independence, and safety in their residence. You are the advocate for each of the members in your caseload throughout the members lifetime in Fortified Life.
- Stay informed about current trends and best practices in wellness and nutrition, and incorporate new ideas into programming."
- Collaborates with many different departments within Western Home to provide the best coaching care for members in Fortified Life.
- Assists when called upon to instruct classes or provide educational wellness opportunities to members within Fortified Life or members within the Western Home Communities and the Grosse Aquatic and Wellness Center.
Requirements:
- Bachelor's degree in nutrition, wellness, health promotion, or related field. Master's degree preferred.
- Diverse health and well-being experience is highly recommended
- Strong case management skills with an aging population is encouraged.
- CPR/AED certified or willing to obtain
- Must be in good general health and demonstrate emotional stability
- Strong understanding of the dimensions of wellness and their impact on overall health and well-being.
