Busy medical practice is seeking a Case Manager for our Healthcare for the Homeless program. The Case Manager will be responsible for screening and assessment: problem identification; care planning; implementation and coordination of care, and monitoring the care process of clients. Effectively assists homeless individuals through intensive outreach and on-site case management services to achieve independence and permanent housing.
Qualifications:
BS Degree in Health Science Education or experience determined to be equivalent.
1-3 years of experience in the field or in related work experience.
Must be familiar with a variety of the field’s concepts, practices, and procedures.
Must be able to communicate effectively in writing, verbally, and before an audience.
Ability to organize and coordinate a variety of tasks and responsibilities.
Must have a valid Alabama Driver’s License and willing to travel as needed.
Summary of Duties:
Accepts patient referral via WHSI providers, other staff, patient request or other agencies request.
Provides assessments, develops care plans and arrange for the provision of identified needs on-site, through referrals or by direct outreach contacts.
Provides on-going case management for assigned case load of patients served at WHSI. Provides follow-up via on-going assessment, evaluations progress toward defined goals, amends care plan as needed and address issues of patient non-compliance.
Provides alcohol and substance abuse assessments, HIV pre and post testing counseling.
Provides support service such as transportation and personal hygiene kits.
Provides patient referrals for financial assistance, medical specialties, shelters, vocational services, legal services, and substance abuse treatment providers, Social Security Administration, Food Stamp Office, Medicaid and other available resources.
