Busy medical practice is seeking a Case Manager for our Healthcare for the Homeless program. The Case Manager will be responsible for screening and assessment: problem identification; care planning; implementation and coordination of care, and monitoring the care process of clients. Effectively assists homeless individuals through intensive outreach and on-site case management services to achieve independence and permanent housing.
Qualifications:
- BS Degree in Health Science Education or experience determined to be equivalent.
- 1-3 years of experience in the field or in related work experience.
- Must be familiar with a variety of the field’s concepts, practices, and procedures.
- Must be able to communicate effectively in writing, verbally, and before an audience.
- Ability to organize and coordinate a variety of tasks and responsibilities.
- Must have a valid Alabama Driver’s License and willing to travel as needed.
Summary of Duties:
- Accepts patient referral via WHSI providers, other staff, patient request or other agencies request.
- Provides assessments, develops care plans and arrange for the provision of identified needs on-site, through referrals or by direct outreach contacts.
- Provides on-going case management for assigned case load of patients served at WHSI. Provides follow-up via on-going assessment, evaluations progress toward defined goals, amends care plan as needed and address issues of patient non-compliance.
- Provides alcohol and substance abuse assessments, HIV pre and post testing counseling.
- Provides support service such as transportation and personal hygiene kits.
- Provides patient referrals for financial assistance, medical specialties, shelters, vocational services, legal services, and substance abuse treatment providers, Social Security Administration, Food Stamp Office, Medicaid and other available resources.
