What are the responsibilities and job description for the Physician position at Whole Foods?
Responsible for the medical care of patients as well as working closely with nurses and health coaches to offer the best patient care experience and improve clinical outcomes.
Responsibilities:

Collaborates with the Medical Director to ensure the company clinic offers the best possible care.

Responsible for the delivery of WFM's care model whether by face-to-face visits (store, Clinic, or the patient's home), by phone, or by email.

Practices "Shared Decision Making" as defined in the Clinic's care model.

Comfortable working collaboratively as part of a team with the patient while maintaining overall clinical supervision of the nurse practitioners, health coaches, and medical assistants.

Serves as clinical supervisor for clinic Team Members, including nurses, health coaches, and other members of the Care Team; may be required to take calls from clinic Team Members at various times during the day.

Serves as primary provider for any High Intensity patients (for example, anyone with a high iHUI score, chronic disease, multiple medications, requiring a specialist, recently or frequently hospitalized, considered for surgery or other specialized testing / imaging / or procedures, or anyone else deemed High Risk by the Physician).

Reviews and approves all:

Medications off of the formulary along with the Medical Director,

Referrals,
-
Surgeries,
-
Specialized testing or imaging,
-
Other procedures performed outside of the Clinic.

Attends and leads daily and weekly Huddles with Care Team Team Members to discuss specific patient problems or progress.

Provides direct patient care as required or as directed by the Clinic's Medical Director.

Assists the Care Team in quality improvement ideas and strategies for more efficient patient care.

Accountable for ensuring all expenditures provide the highest quality care in the most efficient manner.

Available after-hours to assist with on-call.

Maintains the confidentiality of all patient information.

Provides regular medical and nutritional training to the nurses and health coaches.

Contributes to the operational and clinical improvement of the Clinic's care model.

Delivers presentations to Team Members and project stakeholders.

Assists with creating a network of like-minded professionals.

Establishes relationships with hospitalists and emergency department providers, etc.

Education & Experience:
- Board certified in Internal Medicine or Family Practice / Medicine.
- Currently holds an active, unrestrictive Texas Medical License or in the process of obtaining the license.
- Currently hold an active, unrestricted DPS Controlled Substance Registration and DEA Controlled Substance Registration.
