Woods Services is a Pennsylvania and New Jersey-based non-profit population health network and advocacy organization that along with its six affiliate organizations provides life-long innovative, comprehensive and integrated health, education, housing, workforce, behavioral health and case management services to more than 22,000 children and adults in the intellectual and developmental disability, child welfare, behavioral and brain trauma public health sectors who have complex and intensive medical and behavioral healthcare needs. Our Langhorne campus is home to 550 residents, and provides outpatient and day programs.
- The nurse practitioner provides primary care services to individuals served by Woods Services, Inc. with developmental and intellectual disabilities.
- The nurse practitioner will also provide medical care and health assessments to Woods staff in the employee health department.
- Responsible for providing preventive and acute care, chronic disease care, health education and care coordination, and health screenings as appropriate.
- The nurse practitioner is responsible for documenting client notes in the EHR according to set standards and attending IDT meetings.
- Provides primary care services, including medication management, and prescribing medications as appropriate to individuals served at Woods and staff seeking medical care.
- Evaluates and orders appropriate lab/diagnostic tests, chronic health assessments, and coordinate all follow-up care as needed.
- Provides mandated screenings as well as screenings for common chronic conditions.
- Enters all patient services in electronic medical record.
- Provides first aid for illness or injury to Woods clients and document nursing assessment, diagnosis and treatment.
- Establishes health care plans of individuals served by Woods and update them as needed.
- Maintains comprehensive health records on individuals served by Woods.
- Completes peer review reports as required.
- Performs initial or annual employee physicals and drug testing as indicated.
- Ensures that all required reports are properly prepared and submitted.
- Performs all documentation for billing in accordance with agency policy.
- Works collaboratively with Medical Director, Health Center Leadership and delegates to comply with agency policy related to billing, credentialing, and reporting.
- Ensures provision of all clinical services according to the policies and procedures, applicable professional standards, facility and program licensure as required, laws, rules and regulations.
- Facilitates close communication with Woods leadership, management and other key staff for care coordination as appropriate.
- Attends required meetings.
- Provides coverage for other MD/DO/NPs/PAs as necessary.
- Works with the Woods Leadership to develop and actualize strategic vision related to Woods Health Center.
- Supports and facilitates all community partnerships.
- Represents the organization and its partners externally.
- Collectively participate with on call after hours coverage of urgent calls.

EDUCATION AND EXPERIENCE:
- Bachelors Degree in Nursing (BSN)
- Masters Degree in Nursing (MSN) and/or Doctor of Nursing Practice (DNP)
- Five or more years of clinical experience as a Nurse Practitioner.
- Experience working with vulnerable populations.
- Experience working with people with intellectual and/or developmental disabilities preferred.
QUALIFICATIONS:
- Licensed Nurse Practitioner in Pennsylvania
- Board Certification as a Nurse Practitioner in family practice
- Pennsylvania Registered Nurse license
- CPR/AED certification
- Required Act 48 continuing education course work for Nursing license
- Required State and Federal clearances
- Medicaid credentialed and registered as a CRNP with prescriptive authority in PA preferred.
