What are the responsibilities and job description for the Hospice Admissions Nurse position at Wren Hospice?
Wren Hospice is looking for a Admissions Nurse, RN with excellent nursing skills/judgement and understanding of hospice philosophy. The Admissions Nurse will perform initial assessments of patient's needs, serves as a clinical resource for referral sources. The admission nurse responsibility is to present Wren Hospice services and philosophy to patients and families with the primary goal of providing a safe transition onto hospice services. The Admission Nurse develops transition plans that are patient-centered, promoting quality and efficiency in the delivery of hospice care. The Admission Nurse represents the agency in activities involving professional contacts with patients/families, physicians, hospital/facilities, senior living communities, and professional associations to educate them on the availability of Wren Hospice services. The Admissions Nurse will handle acute care visits. Administration on call rotation is required.
