The Clinical Coordinator is an integral care team member and designated clinic lead who serves as point person for clinic operations issues and as a consistent initial clinical contact for new patients and their families. The CC performs initial patient assessments, provides education and navigates the patient/family across care settings according to the established plan of care. The CC synchronizes patient/family care across care settings from the initial referral and consultative visit to handoff to Physician Practice Coordinator, Infusion nursing staff or other facility depending on care plan.

Responsibilities

  • 1. Serves as clinic point person for identification and resolution or escalation of issues arising in day-to-day practice, ensures assignments and daily team meetings/huddles, anticipates delays and facilitates timely forward flow of patients during daily clinic operations.
  • 2. Regularly reviews new patient referrals, assists Intake Specialist in determining initial consultation scheduling priorities and confirms that data collection is complete prior to visit. Collects/documents patient data for new patients and completes Initial Nursing Assessment during pre-visit patient contacts and new patient visits. Initiates survivorship care plans on eligible patients.
  • 3. Develops a patient/family education plan based on initial assessment findings and relevant to teaching/ learning topics and styles and identified special needs and preferences. Documents/communicates to team members.
  • 4. Uses findings of assessment data, team-based interactions and established triggers to initiate referrals for supportive care. Liaises across care settings to ensure familiarity with patient case and special needs; ensures that patient safety needs are met during handoffs to point of care staff (eg, infusion, surgery, therapeutic radiation). Anticipates and/or identifies potential logistical, financial, personal barriers to patient adherence to visit schedule and therapeutic plan; develops a plan to minimize or eliminate obstacles and refers to appropriate team members to provide ongoing support.
  • 5. Assists in the identification of candidates for open clinical trials and communicates to providers and the research team. Has familiarity with and promotes clinical trials and research activities.
  • 6. Sets and strives to meet annual goals for professional development. Maintains membership in at least one professional organization as appropriate. Achieves/maintains oncology nursing certification. Actively participates in YNHH-L M and care center committees and projects
  • 7. Performs other duties as assigned or directed to ensure smooth operation of the department/unit.

Qualifications

EDUCATION

Bachelor's degree in nursing required; Masters preferred. CT/RI RN Licensure

EXPERIENCE

A minimum of three years experience in adult inpatient services, ambulatory oncology service (preferred)

LICENSURE

Active CT RN license, OCN certification preferred. BLS Required.

SPECIAL SKILLS

Excellent interpersonal and communication (oral and written) skills required. Knowledge of adult learning competencies and patient teaching skills. Ability to work autonomously and maintain flexibility.
