Role
Under general supervision, performs administrative functions and coordinates the assigned program for Community Health Center of Yavapai (CHCY). Assigned programs include Care Coordination, Clinical Services, Well Woman Health Check, and 340B Pharmacy.

Major Duties, Responsibilities
Participates in development of protocols, policies, and practices that improve the effectiveness and efficiency of the program; contributes to opportunities to expand services.
Participates in recruitment, hiring, training, supervising, and evaluating staff to promote the mission of the department.
Collaborates with Compliance Officer and senior management to maintain program compliance. Performs program audits to ensure all established requirements and guidelines are met.
Works cooperatively with providers to coordinate the care of patients with specific diagnoses by identifying problems/barriers, appropriate actions/steps and timelines for completing health care goals.
Compiles monthly and quarterly reports, to include a narrative and analysis of statistics for contributing to quality measures and program reporting requirements.
Establishes and maintains community partnerships to improve community health.
Serves as a liaison with community partners who assist with program delivery services; represent the department at meetings and events as needed.
Performs other job-related duties as assigned.
Minimum Qualifications
Education:
High school diploma or equivalent.
Experience:
A minimum of three (3) years’ experience in the medical field with two (2) years’ experience in a supervisory role.
Additional Requirements:
Must possess a valid Arizona driver’s license.

Assist in the development and writing of grant applications, monitoring budgets, and annual plans.
Keep up to date with program changes through training, meetings, and written/electronic communication.
Travel amongst CHCY sites as needed.
Work with supervisor to propose changes to protocols, policies, or practices that improve the effectiveness and efficiency of the program.
Develop and maintain effective working relationships with associates, employees of other departments, representatives of other organizations, County officials, and the public.
Keep management informed of key operating issues affecting the department.
Remain current in knowledge required to perform assigned duties.
Handle confidential matters and maintain discretion always.
Work professionally and effectively with co-workers, and the general public.
Act with courtesy, tact, and diplomacy
Work Environment:
The work environment is associated with an office. The work area is adequately lighted, heated or cooled and ventilated.
Physical Requirements:
Ability to sit or stand for extended periods of time. Sufficient vision to read printed materials and a computer screen; hearing and speech ability to communicate effective in-person and telephonically. Capable of frequent repetitive movement of arms, fingers, and wrists. Ability to lift up to 30 pounds.
