What are the responsibilities and job description for the CNA/Homemaker position at York Hospital?

We currently have a an opening for an experienced CNA/Homemaker. The Home Care Office is located in York, Maine.
The CNA/Homemaker is a professional Certified Nursing Assistant whose primary function is the personal care of the patient. The services of a Homemaker/CNA are given under the supervision of a registered professional nurse or therapist. The specific personal care for each patient will be determined in conjunction with the patient by the registered nurse and therapist, and will be given in writing to the home health aide. CNA/Homemaker will be available for service during the programs usual hours of operation.
In order to help us continue to provide exceptional patient experiences, you must have the following:
Minimum (1) or more years of Home Care experience.
Currently active on the Maine Board of CNA Registry.
Have valid Driver's License.
BLS Certification
Exceptional customer service skills.
