Responsibilities

Program Overview

 

Since 1999, LifeSet has helped more than 8,000 young people aging out of state custody or other care arrangements successfully transition to independence. Participation in the LifeSet program is voluntary. Specially trained LifeSet specialists meet with LifeSet participants at least once a week-and more often when needed-in community settings, including their homes, at school or on the job, at a doctor's office or wherever is most convenient for the young person. Specialists are available 24/7 to help the young adult. Young people typically participate in the program for 6 to 12 months, based on individual needs.

 

As a LifeSet Specialist, one provides individual counseling and case management services to clients in the home and community setting to assist clients with transitioning to adulthood, specifically obtaining and maintaining safe and suitable housing, continuing their education, learning independent living skills, obtaining and maintaining employment, building a support system and remaining free from legal involvement.  

Position Overview

  • Carry a caseload of 10-12 young adults
  • Provide individuals therapeutic treatment interventions for assigned caseloads and provide case management
  • Conduct individual/cognitive interventions with the primary objective being to teach the young adults to think for him or herself. A specialist provides individual cognitive behavioral therapy to client as needed
  • Meet with client a minimum of one time per week, increasing and decreasing as directed by supervisor on the basis of clinical need
  • Conduct on-going assessment of the clients' needs from a strength focused, solution based perspective
  • Maintain a flexible schedule that is convenient for the client in order to meet clients' needs
  • Monitor the quality of all services performed for the client by outside agencies, coordinating services with the treatment plan and teaching the young adult to attain and monitor series after treatment is completed
  • Provide on-call crisis support to the client M-F 24hrs/day, On-Call weekend rotations

 

Additional Information

  • Schedule is flexible and non-traditional; based around the availability of youth and families served
  • Applicants must have a valid driver’s license, a personal vehicle to use for work purposes and auto insurance
  • Community Based staff will be reimbursed for applicable mileage
  • If clinical licensure hours are needed (LPC, LCSW,etc.), this position qualifies for gaining those as you work with young people

 

Salary

$47000 - $54000 / year based on education and clinical license

Qualifications

Requirements

  • A Bachelor's degree in social services discipline with experience working with at-risk youth, adults, or families is required
  • A Master's degree in a social services discipline is preferred
  • Degrees that can be considered include: social work, drug and alcohol education or counseling, psychology, criminal justice, guidance counseling, or marriage and family therapy. Other degrees will have to be evaluated further to determine if they meet the qualifications
  • At least 6 months of related experience in counseling and/or case management is required
  • Strong organizational skills and attention to detail
  • Excellent written, verbal, and oral skills
  • Ability to manage multiple priorities simultaneously
  • Basic computer knowledge
  • Ability to maintain a flexible schedule
