Zepf Center has been serving the Lucas County community for over 40 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, and therapy programs, as well as career development and wellness services. Zepf Center partners with Neighborhood Health Association to provide primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.
We are currently seeking a full-time Act Team Leader to manage both clinical and operational oversight of an integrated care group (ACT) in the Care Coordination Department.
Essential Duties of this position include but are not limited to:
- Maintain compliance of all integrated care/crisis plans while providing clients with the highest quality of care.
- Reviews and assigns eligible patients to the ACT team.
- Serves on agency committees/teams, attends meetings internal and external of the agency.
- Complete Assessments on patients in accordance to ACT compliance
- Some on Call
Requirements
- LISW, LPCC, CNP, APN required. If Dependent, must receive independent within 24 months.
- Degree in Social Work or related field with at least 5 years of supervisory experience preferred
- Health management experience and an understanding of data management, managed care and quality improvement strongly preferred.
- Must possess a basic understanding level of electronic health records and have excellent computer skills.
