Zepf Center has been serving the Lucas County community for nearly 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.
Summary:
- The purpose of this position is to effectively monitor, track and support specific quality improvement initiatives including 7-day follow-up after psychiatric hospitalization.
- Promote continuity of care between hospital and community settings.
- Develop and maintains good working relationships with hospital discharge planners and/or social workers and the Emergency Department for effective care coordination.
- This position is working in the hospital setting, office and community.
- This position requires strong problem-solving skills, coordination of behavioral and physical health and social services and coordination with the care management staff.
- Future growth of this team may result in this position having direct reports to supervise.
Essential Duties and Responsibilities:
- Reviews and assesses hospital admissions and discharges daily. Engages in regular collaboration meetings with Nursing Manager and primary care staff related to hospitalizations and post discharge needs.
- Coordinates services between hospital and Zepf Center staff (May include completing an initial assessment).
- Attends hospital stuffings consistently and maintains regular contact with same staff.
- Consults with attending doctors in order to promote continuity of care and facilitate discharge.
- Facilitates gathering new patient information and/or updating existing information
- Client status update forms & demographic changes
- Initial Income Verifications and Financial Updates
- Actively participates in discharge planning and development of after care services. Consults and provides ongoing progress updates to the assigned staff.
- Meets with client at their preferred location, within 7 days of hospital discharge to complete an updated DLA or DAU assessment and support the successful transition back to outpatient services.
- Completes all necessary documentation per agency standards.
- Notify clinical staff regarding hospital discharges of their assigned clients.
- Respond to client phone calls and investigate client inquiries as necessary.
- Carry out policies and procedures of the agency; may participate in community work, staff development, agency-wide projects; serve on agency committees/teams including quality and value-based initiatives, attend meetings and perform other miscellaneous duties as requested.
Requirements
Position Qualifications:
- Ohio License required (LSW, RN) with minimum of 5 years’ experience in progressive behavioral health organization, with at least three years’ experience overseeing care coordination activities.
- Must be or have the ability to become a Mental Health Officer within 6 months.
- Must have excellent written and oral communication skills and computer proficiency.
- Supervisory experience preferred.
