The LPN collects and analyzes data from the patient's medical history, nursing assessments and staff input on an ongoing basis. The LPN provides as needed, crisis care, transitional, or vigil care visits as assigned. All care directly provided or coordinated by the LPN is in conformance with federal/state regulations.
