The team is comprised of navigators, RNs, Advanced Practice clinicians and a board-certified Emergency Medicine physician. This collaborative group works to take care of patients, who are outside of the hospital setting, in the moment or helps direct the patient to the most appropriate level of care. Through many different access points, patients are able to reach the team 24x7x365 to help with medical issues in real time. The vAcute team works collaboratively with the patient’s Primary care physician and specialists to optimize the patient experience, increase communication, and ensure the patient receives care in the most appropriate setting.
