Job Description Job Description Assertive Community Treatment (ACT) Team Clinician: We are seeking a compassionate and skilled Assertive Community Treatment (ACT) Team Clinician to provide outreach, therapy, and clinical case management services to individuals with severe and persistent mental illness (SPMI). This role will also support individuals with co-occurring substance use disorders, delivering integrated care to help them achieve stability and live successfully in their homes and communities. The ACT Team Clinician will collaborate with a multidisciplinary team to develop and implement personalized care plans, ensuring that everyone receives the comprehensive support they need to manage their mental health and substance use challenges effectively. This position requires an individual who is proactive, empathetic, and committed to empowering clients to reach their full potential while living independently.
