Overview: This position will cover Moore, Lee, and Chatham Counties. Will be conducting a range of activities from administrative tasks to field work. This position will work with assigned patients and help the patient navigate the healthcare system to achieve outcomes, assist the lead in developing care plans with patients; conduct home visits, visit patients in the hospital and conduct follow-up visits by telephone with patient, help advance patient self-management through personal contact and tools, and assist team members in the development of new strategies for health & social care. Associates degree in public health, community health or comparable degree required, Bachelors degree preferred. Work experience in related field can be accepted in lieu of degree. Valid NC Driver's License with good driving record required.

Position Summary: Obtains blood specimens by venipuncture and capillary methods, for a variety of laboratory tests and procedures to be used in diagnosis and treatment of disease. Provides patient education on collection procedures. Requirements: High school diploma or equivalent required. Completion of an accredited Medical Assistant Program, Phlebotomy Program or a minimum of one year of phlebotomy experience required. ASCP or equivalent certification preferred. Eligible for certification required.

SLP will be responsible for conducting diagnostic speech, cognition and swallowing evaluations for adult and geriatric patients.
