Transportation includes ensuring individuals to ensure that they will be connected to care provided in the community setting, assisting individuals to the vehicle as needed, transporting individuals to the bus station, medical hospital, their residence or any other designated location. While majority of individuals are independent with transfers and ambulation, some may require additional help to safely transfer in and out the car. Assisting individuals who are using wheelchair may also be required.
