Depending on the number of Medicaid enrollees in the counties it services a CMHSP may be a "stand alone PIHP" or if the number of persons is below 20,000, CMHSPs may join together in an Affiliation. The Northwest CMH Affiliation includes two organizations: Northern Lakes CMH Authority (which holds the contract with the Michigan Department of Community Health and is also doing business as the Northwest CMH Affiliation to serve as the PIHP) and West Michigan CMH System. The geographic area of Northern Lakes CMH includes the six counties of Crawford, Grand Traverse, Leelanau, Missaukee, Roscommon, and Wexford. The geographic area of West Michigan CMH System includes the three counties of Lake, Mason, and Oceana.
