Requirements and Responsibilities

Case Manager - Home Care/Hospice coordinates the overall care plan for home health/hospice patients. Evaluates the needs of the patient, the resources available, and recommends the plan for the best outcome. Being a Case Manager - Home Care/Hospice collaborates with physicians and other medical staff members to facilitate the progression of care. Monitors the patient's progress and acts as a liaison between the patient, medical staff, and insurers. Additionally, Case Manager - Home Care/Hospice identifies patient care issues and makes recommendations. Typically requires a bachelor's degree in nursing. Requires a Registered Nurse (RN) license. Typically reports to a manager. Case Manager - Home Care/Hospice's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement.
