The Hurlbut, a 160 bed skilled nursing facility in Rochester, is looking for well qualified RN MDS Coordinators to join our team Full Time .

Under limited supervision, develops, completes, and transmits resident assessments (MDS) in accordance with current Federal and State standards. Applies comprehensive knowledge of MDS processes, performs utilization reviews, and has knowledge of Quality Improvement and Care Planning.
Required Skills/Responsibilities:
· Demonstrated leadership ability.
· Proficient communication skills, both oral and written
· Proficient computer skills and knowledge
· Proficient in data collection and analysis
· Detail-oriented and able to operate with minimal oversight
· Works independently as part of the Interdisciplinary Team (IDT)
· Organized and able to prioritize and submit timely assessments
· Demonstrate clinical competence - Knowledge of Nursing and medical practice and procedures
· Prepares MDS schedule within CMS guidelines, and distributes to IDT
· Audits completed MDS and reviews validation reports to track errors.
· Maintains up-to-date knowledge of MDS, Federal and State regulation/requirements
· Participates in Team Meetings for upcoming MDS reviews
· Provides in-service education for team members/new hires as needed
· Participates in Quarterly Corporate MDS Meetings and keeps current with evidence-based practices in the field
Required Experience
· Current New York RN license (will consider LPN with proven experience)
· Current CPR certification
· Two years’ clinical experience in an acute care or post-acute care facility
Preferred:
· Prior experience in an acute care facility
· Prior experience in long-term care setting
· Prior MDS experience
· Knowledge of OBRA, PDPM PPS MDS, and Medicare regulations
· Prior experience with Quality Measures
