LCSW Social Worker

• Provides clinical support to ECM care coordinators and clients enrolled in Enhanced Care Management services. This includes chronic disease management.
• Engages vulnerable populations as part of a multidisciplinary outreach team. This includes home visits, accompaniment to appointments, outreach to hospitals, homeless shelters and other settings, as needed.
• Provides clinical oversight and tracking for comprehensive intake assessments.
• Participates in developing patient-centered care plans for the enrollees on their panel. Approves initial plan and bi-annual updates.
• Helps to expand programming that addresses SDOH and enhances connections to community-based organizations.
• Works with hospitals to coordinate hospital admission/discharge plans with the behavioral health clinician, PCP, pertinent specialists, and other organizations with the goal of preventing readmission, if possible.
• Uses data to evaluate outcomes from targeted interventions for all patients. Assists in developing appropriate adjustments to care plans based on this data.
• Implements prevention & engagement activities.
• Provides health coaching for groups of enrollees or individual enrollees as needed.
• Engages in quality improvement efforts.
• Guiding Care Coordinators in steps to ensure the member’s safety and providing immediate consultation and guidance for any situations needing escalation of care.
• Monitoring continuous quality improvement measures for coordination of care through documentation review, clinical consultations, peer review, and other related activities to ensure quality of services.
• Overseeing charting and documentation standards adhering to ECM contracts with Managed Care Plans (MCPs).

Required:

• Valid LCSW license in California.
• A minimum of one year of experience working with vulnerable populations, including persons with a history of trauma, those experiencing socioeconomic stress, homelessness, or substance use disorders.
• Strong interpersonal skills and clinical problem solving.
• Comfort working in an autonomous clinical and outreach setting.
• Capability of working with data to track and measure performance.
• Basic computer skills, including the ability to send and receive emails and summarize data in spreadsheets.

Provide clinical oversight of ECM services by:

• Provide staff development training for ECM Care Coordinators.
• Ensure that ECM strategies and services are whole- person centered, linguistically, and culturally appropriate.
• Guide Care Coordinators in steps to ensure the member’s safety and providing immediate consultation and guidance for any situations needing escalation of care.
• Monitor continuous quality improvement measures for coordination of care through documentation review, clinical consultations, peer review, and other related activities to ensure quality of services.
