Medical Social Worker (MSW) - Home Care provides social work services to home care patients and their families. Assesses and treats emotional and behavioral problems related to patient illness. Being a Medical Social Worker (MSW) - Home Care participates as a member of multi-disciplinary team and may assist patients in treatment planning. May contribute to the formulation of discharge plans. Additionally, Medical Social Worker (MSW) - Home Care requires a master's degree of social work. Typically reports to a senior case manager. Medical Social Worker (MSW) - Home Care's years of experience requirement may be unspecified. Certification and/or licensing in the position's specialty is the main requirement. Responsibilities

*Coordinating the care of patients with complex needs that require multiple services over time. *Market and promote business services to various physicians, attorneys, rehabilitative centers, physical therapist, discharge coordinators, outpatient centers, nursing homes,. home health agencies and administrative staff to care for all patients of all ages, from infants to the elderly as well as their families. *Assisting patients with understanding financial and emotional resources available to them *Reviewing new admissions for any special needs or issues that need to be addressed.
Qualifications

*Bachelor's degree in social work, sociology, or psychology *Minimum 3 years experience
