Nurse Practitioner - Specialty Care jobs in Pasadena, CA

We are a Mobile Clinic that visits patient's home, board and care and assisted living facilities.

Candidate will do the following :

• You will be assigned to a City of your choice or near your Home/ Work covering multiple locations and working with different teams to manage patients at their homes
• You will deliver proactive preventive care at patients' homes to reduce unnecessary hospitalizations visits.
• Partner with the patient's primary care provider and specialists to deliver integrated care

• Diagnose and treat patients, consistent with standard of care
• You will be conducting comprehensive exams and creating care plans for the most vulnerable, senior elderly Medicare patients
• Providing appropriate education and counseling to patients and their families/caregivers
• Documenting appropriately in EHR and other systems
• Collaborating with other members of the care team to manage the needs of the patient

Job Types: Full-time, Part-time, Contract, PRN, Per diem

Pay: $160.00 - $180.00 per hour

Expected hours: 1 – 40 per week
