Transplant Nurse Coordinator provides planning and coordination of all facets of clinical care for potential and actual transplant candidates and recipients. Manages referral, evaluation, and pre-transplant data for transplant patients and coordinates the clinical management of post-transplant patients. Being a Transplant Nurse Coordinator develops materials and delivers programs to educate and support patients, families, and clinicians throughout the transplant process. Requires clinical experience in the care and treatment of transplant patients. Additionally, Transplant Nurse Coordinator requires a bachelor's degree of Nursing. Typically reports to a manager or head of a unit/department. Requires registered nurse (RN). May require certification from a professional organ transplant nursing certification board. Reporting to the Director of Quality, the Clinical Quality Coordinator RN will be responsible for overseeing and coordinating all aspects of the Trauma, Stroke, Chest Pain, and other designated Specialty Programs. The Clinical Quality Coordinator RN will work collaboratively with the Specialty Program Medical Director to ensure quality and safe patient care practices are being demonstrated.
The Clinical Quality Coordinator RN will also serve as a patient and family advocate through the continuum of patient care while upholding the mission and values of the organization. In addition, the Clinical Quality Coordinator RN will work closely with staff and Leaders across the organization’s service lines including the ICU, ED, and Observation Units to assist, educate, and provide quality assurance for the departments ensuring all quality requirements are being met.
