This position requires applicant be 18 years of age or older.
Summary of Responsibilities
Under the direct supervision of the Activity Director, assists in providing nursing home residents with a program of planned activities to meet the individual needs and interests of each resident; assists in developing programs and activities; sets up, directs, and assists residents in activity participation; conducts reality orientation and re-motivation groups.
1. Instructs/assists residents in the completion of crafts, woodworking, needle work and other projects, assists in planning and supervising both active and passive activities such as games, movies, sing-alongs, talking books, religious services, etc.
2. Conducts daily individual reality orientation and weekly group activity sessions with residents; encourages residents to participate in available programs and to maintain a positive outlook on life.
3. Compile monthly calendars for assigned unit based on resident’s needs and desires.
4. Prepares activity area daily, setting up projects for residents to work on; sets up for other programs as required, such as craft sales and religious services; assist residents in getting to and from activity areas and restrooms.
5. Maintains records of resident attendance, progress and activity participation.
6. Communicate and interact with family members as to residence’s participation in planned activities.
7. Conduct one-on-one interactions with special needs patients.
8. Attends Care Plan weekly meetings to discuss observations, change in conditions and level of progress to aid in developing plan of care for assigned residents.
9. Organizes and conducts resident outings.
10. May be required to drive the van on outings.
11. Performs other duties as required or assigned.'
