    OBJECTIVE SUMMARY: Serves as a member of the health care team, assisting individuals/families and the treatment team through the provision of psychosocial assessments; advocacy; education; treatment and discharge planning for all patients admitted to the inpatient acute medicine and swing beds; patients admitted to the Emergency Department and through patient self-referral, social work, case-finding and/or referral from medical and nursing staff.  Social Workers will screen all patients to determine extent of social work services needed by each patient.

     

    ESSENTIAL FUNCTIONS:

    • Provides a psychosocial assessment of patient, family or appropriate support systems in order to implement crisis or support interventions and to maximize coordination of services
    • Conducts standardized mental health screenings for patients admitted to the Swing Bed Program
    • Initiates discharge planning with patients and their families to develop a plan that meets the patient’s needs in both short and long term planning. Examples:  coordinates residential placement, transportation, homecare, mental health follow-up
    • Provides counseling/education for patient, family, support systems regarding adjustment to illness and hospitalization, end of life issues, decision-making. Re: substance abuse, domestic violence, elder abuse, and child abuse
    • Resolution or identification of other problems related to the patient’s transition within the health care setting
    • Coordinates, negotiates and advocates for patient’s needs with payers and external case managers
    • Maintains verbal and written communication with the multidisciplinary team on patient’s psychosocial adjustment, interventions and goals through medical record documentation and consultation
    • Participates in social work and hospital-wide quality assurance activities
    • Participates in hospital interdisciplinary activities, such as committees, team planning meetings, informational meetings
    • Assists in the development of Swing Bed group and milieu activities and participates as facilitator in the program as requested
    • Actively participates in own performance review, being open to constructive feedback and to integrating recommendations into practice
    • Maintains current professional license, including requirements for ongoing continuing education


    REQUIRED EDUCATION: Bachelor’s Degree in Social Work

    PREFERREED EDUCATION: Master’s Degree in Social Work

    REQUIRED EXPERIENCE: Supervised experience as a social worker in a hospital setting

    ADDITIONAL ELIGIBILITY QUALIFICATIONS: Registration as a Social Work Associate Advanced (SWAA) or Social Work Associate Independent Clinical (SWAIC).

    • Assesses and assists with non-medical problems interfering with effective patient care
    • It is essential that social work employees be able to work effectively with customers, including patients, visitors, staff, volunteers, physicians and community resources.
    • Each social worker is expected to demonstrate honest and timely communication with respect for the individual. Ability to effectively work with multidisciplinary teams. Continuous quality improvement with customer focus. Innovation and creativity. Adaptability to changing roles.

     

    WORK ENVIRONMENT: Potential exposure to patient infectious disease and body fluids. Function in emergencies and incidents involving patient care


    PHYSICAL DEMANDS: Assist with moving and transferring patients (involves pushing and pulling). Natural or corrected near-visual acuity is necessary to read patient charts, physician orders, computer screens. Natural or corrected hearing is necessary to communicate effectively with others.
