Adult Day Care Director creates and directs programs that address the health, psychosocial, and educational needs of older individuals and their caregivers and that provide clients a safe, secure, and positive daily experience. Responsible for recruiting, training, and retention of qualified staff. Being an Adult Day Care Director manages the financial operations for the center including billing, reimbursements, and budgets. Performs assessments of prospective clients to determine if programs will meet their needs. Additionally, Adult Day Care Director coordinates with caregivers, families, social services agencies, and medical professionals to plan clients program of care. Monitors programs to ensure effectiveness, quality care, compliance with applicable regulations and standards, and to identify areas for improvement or expanded program offerings. May be a RN with experience in gerontology. Requires a bachelor's degree of social science or a directly related field. Typically reports to top management. The Adult Day Care Director typically manages through subordinate managers and professionals in larger groups of moderate complexity. Provides input to strategic decisions that affect the functional area of responsibility. May give input into developing the budget. Capable of resolving escalated issues arising from operations and requiring coordination with other departments. To be an Adult Day Care Director typically requires 3+ years of managerial experience.     Job Title: Care Coordinator

    Job Summary:

    The Care Coordinator provides non-clinical services intended to aid high-risk women and their families in establishing recovery from substance use problems. The Care Coordinator will work as part of the treatment team to provide peer support, mentoring, and motivation to encourage patients to change their substance use behavior, focusing on expectant mothers and women of childbearing age who are at a high-risk of delivering a child with Neonatal Abstinence Syndrome. This includes direct interventions with patients while in the hospital or in the community to help access treatment and other recovery support services during pregnancy and after delivery. The Care Coordinator will see patients in assigned hospital units, doctor offices, at the Southwestern Regional Day Report Center as well as conduct outreach and engagement in the community for patients. The Care Coordinator will assist in the development of service plans to promote successful linkage to referred services and will work closely with Family Treatment Court.

    EDUCATION, EXPERIENCE: High school diploma or G.E.D. is required. The candidate should be actively engaged in his/her own recovery program with a minimum of two years demonstrated personal recovery and sustained abstinence from alcohol and/or drug use. Allies of recovery will be considered for the position with preference given to persons with lived experience with a substance use disorder.

    LICENSE/CERTIFICATION/REGISTRATION: CCAR Certification is preferred. State Licensure as a Peer Recovery Support Specialist preferred.

    SKILLS: The Care Coordinator must demonstrate excellent interpersonal skills, the ability to relate to patients and health professionals, as well as an ability to develop professional working relationships with partner agencies. The candidate must possess excellent listening, verbal, and written communication skills, as well as good problem-solving skills. The incumbent must be able to read, interpret documents, and write routine reports. Basic computer skills are required. Must be able to demonstrate patience and tact when dealing with patients, families, and other staff, to integrate Continuous Quality Improvement principles for service and organization work improvements. Ability to work with people of all social, economic, and cultural backgrounds; be flexible, open-minded and adaptable to change. to develop collaborative relationships with physicians, the courts, families, patients, interdisciplinary team and other community agencies. In addition, the candidate must demonstrate a willingness to learn and an interest in acquiring new skills. The Coordinator must have a valid driver’s license and vehicle to complete the community outreach.

    Primary Duties & Responsibilities: The Care Coordinator will meet the individual in need at the referral site to immediately begin to coordinate care services and provide support to patients. Throughout their participation in treatment, the patient will meet individually with the Care Coordinator to provide mentoring and guidance through the recovery process. The Care Coordinator will report to the treatment staff on a regular basis any concerns or suggestions regarding patients’ risks and needs. The Care Coordinator will meet with health care professionals involved in the patients’ care to ensure good collaboration of services.

    Salary: $23,100.00 annually plus benefits. ($11.10 per hour).
