    • Completion of a Registered Nursing program.
    • At least two years of experience in Case Management, specifically in utilization management, discharge planning, or other cost/quality management roles.

    Job Description:

    The Director of Case Management will oversee the hospital's case management program, focusing on care facilitation, utilization management, case management, and discharge planning.

    Responsibilities:

    • Direct the daily operations of Case Managers and Social Workers, providing leadership and educational support.
    • Ensure the department’s documentation complies with necessary regulatory standards.
    • Collaborate with financial and quality departments to enhance quality improvement programs and analyze data trends such as Avoidable Days and Readmissions.
    • Maintain and enhance skills in case management and utilization review to adequately cover patient caseloads and hospital staffing requirements.
    • Facilitate communication with physicians to aid in the development of patient treatment plans and manage level of care and bed assignments.
    • Handle personnel actions including hiring, performance evaluations, and the management of schedules and payroll records.
    • Lead daily Multidisciplinary Rounds to foster interdisciplinary collaboration for holistic patient care.
    • Engage actively in discharge planning, providing essential education and resources tailored to the needs of patients and their families.
    • Participate in the Utilization Review Committee and Revenue Recycle Committee, aiming to optimize clinical resource utilization.
    • Uphold compliance with all relevant regulations and standards from agencies such as the Joint Commission and CMS.

    Knowledge, Skills & Abilities:

    • Proficient in understanding payer requirements, discharge planning regulations, and the development of operational policies.
    • Knowledgeable about Medicare, managed care, and the continuum of care including inpatient, outpatient, and home health.
    • Ability to collaborate effectively with all levels of healthcare professionals.
    • Skilled in performance improvement methodologies.
    • Strong self-motivation, communication skills, and the capability to work both independently and as part of a team.

    Education:

    • Bachelor of Science in Nursing degree is preferred.

    Experience:

    • Previous management experience of two to three years is preferred, along with a minimum of two years in hospital-based nursing.

    Certificates/Licenses:

    • Mandatory Reporter Certificates for Child and Dependent Adult Abuse in Iowa
    • Current RN license in the state of Iowa or a multistate license valid in Iowa.
