This role focuses on providing in-home healthcare services, supporting patients in maintaining health, independence, and comfort in their homes. Key Responsibilities

• Provide comprehensive home health care services, including assessments, treatments, and care plan development for patients in Derry, PA, and surrounding areas.
• Monitor patient progress, update care plans as necessary, and communicate with the broader healthcare team.
• Document patient information and maintain accurate medical records.
• Provide on-call support for after-hours patient needs, as scheduled.

Requirements

• Licensure: Active RN license in Pennsylvania.
• Experience: At least 1 year of RN experience; home health experience preferred.
• Education: Associate Degree in Nursing (or higher).
• Certifications: BLS/CPR certification (AHA; ACLS accepted as well).
• Additional Requirements:
• Valid driver’s license.
• Reliable transportation with active auto insurance.
• Willingness to travel to patient homes.

Position Details

• Schedule: Monday - Friday, 8:00 AM - 5:00 PM.
• On-Call:
• 1 on-call shift per week (5:00 PM - 10:00 PM).
• 1 weekend per month.
• Travel: Mileage reimbursement provided.
• Onboarding: Comprehensive 6-week program tailored for RNs transitioning to home health care.

Compensation & Benefits

• Bonus: $15,000, paid out quarterly.
